A transsexual is a person whose brain (conscious) sex is opposite to their biological (birth) sex. So:
a) case: born male but with female consciousness (male to female, MTF)
b) case: born in a female body but with a male consciousness
Transsexuality is about gender identity, from which sexual orientation (who you are attracted to) is independent.
Transsexuals include heterosexuals, homosexuals, bisexuals and asexuals, as well as those who share the same brain and body sex (medical terminology calls them cisgender).
For some people, getting in harmony with themselves requires lengthy series of surgeries and lifelong hormone therapy. But this is not a requirement. There are also transsexuals who are comfortable in their physical gender.
